VOLUNTEER FORM

Vulnerable Sector Checks are required for volunteers coaching with St Albert Minor Ball Hockey.
VSC are required when volunteering in a position of authority or trust over vulnerable youth.

Please print the volunteer letter and bring it with you along with two pieces of ID to the local police detachment. Once received please scan or send a photo of your VSC to sambh@telus.net by March 16, 2026.

*If you have a VSC issued in 2025 it will be accepted.
