VOLUNTEER FORM

Background searches are required for volunteer parents co-coaching with St Albert Minor Ball Hockey.

SAMBH will accept CRC from another program if dated within the autumn of the previous year.

Residents who are volunteering do not pay for this service when you bring a copy of the SAMBH Volunteer Letter and take it with you to the police station along with two pieces of ID photo/current address.
